University College Hospital, Ibadan Admission into School of Health Information Management 2017/2018 ND/HND Form

Applications are invited from successful candidates who scored 150 and above in the 2017 UTME (ND applicants) for admission into the School of Health Information Management of the University College Hospital, Ibadan for aTwo—Year Programme leading to the award of the Notional Diploma (ND) and also a Two-Year Higher National Diploma (HND) Programme in Health Information Management. The Course, which is non-residential, shall commence in October 2017.
 
1. ENTRY QUALIFICATION FOR ND PROGRAMME 
Apart from the UTME result, applicants must have obtained the minimum entry requirements of five (5) Credits at not more than two sittings in SSCE./GCE/WASC/NECO in the following subjects: 
(a) English Language 
(b) Mathematics 
(c) Biology 
(d) Physics  or Chemistry 
(e) Any other subject,.

2. CONDITIONS FOR ADMISSION (HND PROGRAMME) 
Selection of candidates will be on the basis of good academic qualification, good performance in the Computer Based Test (CBT), and oral interview as well as presentation of relevant ORIGINAL CERTIFICATES. 
 
Applicants with Merit or Pass in their National Diploma (ND) MUST ALSO possess the ND Practicing License or three (3) Years Work Experience 
 
The Computer Based Test (CBT) for HND will be in: 
(a) English Language 
(b) Health Records Management 
(c) Anatomy & Physiology 
Please note that: 
i Pregnancy is not allowed during training 
ii. Successful applicants will be required to first undergo and pass a medical examination a tler admission. 
 
3. METHOD OF APPLICATION 
Interested ND applicants are to pay a non-refundable fee of N2,500 as fee for screening, while HND applicants are to pay N7,500, as fee for Application Form and Screening, into the UCH TSA ACCOUNT (TSA/UCH/SHIM/ADMISSION FORM) in any commercial bank. Applicants are expected to convert their bank tellers into receipts at the Cash Office of the Finance & Accounts Department, University College Hospital, Ibadan, Photocopies of the bank teller and receipt should be forwarded/ brought to the Administrative Office of the School. 
 
All applicants must have functional Email addresses and are expected to collect their Access Code from the School's Administrative Office to apply online on: www.shim.uch-ibadan.org.ng. Application must be completed online not later than 28th August, 2017. 
 
The screening exercise for the ND programme will be conducted on Wednesday 13th September, 2017, while the HND Entrance Examination will hold on Tuesday 5th September, 2017 at the Smart Classroom Two (2), Second Floor University College Hospital, Ibadan by 10am
 
Successful applicants will be invited for an oral interview at a later date through text messages and the School's website.
